To add S&P500 symbol on the MT5 S&P 500 Trading iPhone App - follow the procedure explained below:

On the MT5 iPhone App - Tap the '+' button on the top-right corner of MT5 iPhone S&P 500 iPhone Trading App
Select a Group of Stock Index Symbols From this MT5 iPhone App
Click on the S&P500 Quote that you want to add to MT5 S&P 500 iPhone App
After S&P500 Symbol is added to the Index Trading Symbols Tab on the MT5 S&P iPhone App - then Click the back button

This will add the S&P500 Trading Quote that you want to trade on to MT5 S&P 500 Trading iPhone App.
